Registration Process Information

If Los Banos Jr. High School is your residence school boundary, parent/guardian can pick up a paper copy of enrollment packet in our office, or you can start the enrollment process online and attach all documents needed – see list below.

Required Documentation

  1. Birth Certificate

  2. Immunization Card/Shot records (out of country immunization must visit health clinic for update)

  3. Current withdrawal grades and checkout sheet, if started current year at another school

  4. Last Report Card

  5. Proof of Residence (PG&E, Telephone, water bill, etc.)

  6. Current IEP for Special Education, Speech or 504, if applicable

  7. Legal paperwork such as Custody Orders, Adoption Paperwork, Name Change, etc.

Withdrawals

Please contact Registrar in advance in order to prepare paperwork and obtain required signatures.
